Meet the Team: Director of Care Coordination

As Director of Care Coordination for Care at Home, Stephanie oversees the client’s home care experience. Her responsibilities include managing personalized care plans and ensuring skilled caregivers are matched appropriately to clients. Stephanie also supervises the Scheduling Team to maintain the highest level of caregiver consistency and the Field Technicians to confirm clients and caregivers are developing meaningful bonds. More importantly, Stephanie ensures both client and family care expectations are delivered.

Stephanie joined Care at Home in 2022 and brings a wealth of experience to the team. She has served the state of New Hampshire as a Child Protection Worker and a Case Manager for people struggling with addiction. Stephanie eventually gravitated to serving the elder community. Her passion for home care stemmed from living with her grandmother.

Throughout childhood, Stephanie and her sisters helped care for their grandmother. Home care agencies also assisted the family with respite and hourly care. Stephanie recognized the positive impact home care services had on her grandmother’s quality of life and how it enabled her to remain in the family home for many years.

Stephanie lives in Connecticut with her husband and three-year-old son, Calvin. She enjoys spending time at the gym and bike riding with her son.
